Information to Improve Your Dental Care Experience

Thank you for giving us the opportunity to serve you! Your first visit will take one hour for the consultation and exam with the dentist. If you have healthy gums and would like a cleaning following the initial visit, well schedule an additional hour with our hygienist. 

To save time when you arrive, fill out these forms, print and bring them to your visit. You may also fill out these forms by hand after printing them. We only need the following filled out: the Patient Info., Insurance info, Health History, HIPPA, Office Policy, and Smile Evaluation.

Our front desk team is well versed in dental insurance, and they will gladly answer all of your questions about coverage, copays, deductibles, and financing. We also offer financing to help you get needed dental treatment, while still maintaining your monthly budget.
